You can download and fill out the intake and consent to treatment forms HERE . Otherwise please arrive 15 minutes early to your appointment time and you can fill out the forms at the clinic.

Your acupuncturist will meet with you to discuss your specific concerns, health history, answer any questions you have, and create a treatment plan based on this. We will also guide you through how the clinic is set up so your next visit will be even easier.

After this you will make your payment and we will get started on your acupuncture treatment and may include other modalities as necessary.

We generally recommend about 30-45 minutes for your treatment. Once you’re resting, your acupuncturist will be accessible if you need anything during your treatment.

Las but not least, you relax and enjoy your treatment!

Clinic Etiquette

  • Be on time. When people are late, it can set the acupuncturist behind and make others wait too long for treatment.
  • Please enter the clinic quietly, silence your phone, and use your “library voice” to help preserve the calm environment.
  • Please don’t wear perfumes or heavy scents.
  • No eating in the treatment room.
  • Do not answer your phone in the treatment room.
  • You are more than welcome to bring ipods or reading material-please keep the volume low enough that it can’t be heard by others and please don’t read loud newspapers.

Clinic Policies

Non-discrimination Policy: Common Roots Acupuncture does not discriminate on the basis of race or national origin, age, religion, ability, education, height, weight, body type, marital status, sexual orientation, sex, or gender identity. Your treatment will never be negatively influenced by any of these factors. All are welcome!

Cancellation Policy: All missed appointments and those cancelled with less than 12 hours notice are subject to a $30 no- show/late-cancellation fee. The online scheduler will not allow you to cancel appointments less than 12 hours prior to the appointment time. You will need to email or call us at 520-276-9555 promptly to cancel or reschedule the appointment.

Help us be Scent-Free! We are a fragrance-free clinic. Please help us to accommodate our patients who are chemically sensitive to fragrances and other scented products. Thank you for not wearing perfume, aftershave, scented hand lotion, fragranced hair products, essential oils, and/or similar products during your visit.


Kids at CRA: We do treat children at CRA provided that they are able to sit quietly during their treatment. Children should be accompanied by an adult. Some caregivers bring books or DVDs to occupy kids during treatment. 15 minutes with the needles is usually plenty of time for a younger child. We find that kids who want to receive treatment usually do fine; if

your child is frightened or just isn’t interested in treatment it is probably better to wait until they are older.

Animals at CRA: We do NOT treat pets at CRA and do to patient sensitivities ask that you not bring your pet in with you, unless a service animal.

Closing time: The clinic will close one hour after the last appointment time. Please keep this in mind if you like to sleep for a long time during treatment!
