You can print out your new patient intake forms here, fill them out, and bring them with you to your first appointment.

Otherwise please arrive 15 minutes early and fill out the forms at the clinic.

LOCATION

Common Roots Acupuncture is operated out of Rooted Integrative Wellness. Rooted is located at: 1600 North Tucson Blvd, Suite 100, Tucson, AZ 85716. The entrance is on the South side of the building, which is on Lee st. There is plenty of accessible parking either in the parking lot or along Lee st.

HOURS

Patient appointments are available on Sundays from 10am- 3:30pm and Wednesdays from 3pm- 8:30pm. The easiest way to schedule is through our online scheduler. You can also email or give us a call as well. If the schedule is full and there are no open appointments on the day you would like to come, email us at commonrootsacupuncture@gmail.com or give us a call and we can check for additional availability.
